This position focuses on outreach and linkage to care for medically underserved individuals in Benton and Walla Walla counties. The care coordinator will also accept referrals from the Greater Health Now (GHN) Community Hub, for subsequent support for their medical and related needs. The care coordinator will work in a variety of settings in the field to maximize opportunities for finding people who need more medical support and linkage to primary or specialist care, insurance coverage, or social services. This is a full-time, permanent position. Starting hourly range for this position is $25–$26.50 per hour. This position is eligible for full BMHTH benefits.

Duties:

  • Ensure confidentiality of all Blue Mountain Heart to Heart (BMHTH) clients, current and former
  • Provide outreach to medically underserved individuals and offer linkages to appropriate care
  • Receive referrals from the Community Hub, and respond to these referrals as appropriate
  • Provide timely, accurate, and complete reporting on cases in electronic health records systems, subject to regular review by supervisory staff, funding organization staff, and/or Executive Director or their designee
  • Conduct outreach to potential partner organizations so that field work will have adequate opportunities for identifying individuals to help in the program
  • Liaise regularly with program stakeholders at GHN and around the community, and communicate with them and supervisors regarding client and program progress
  • Attend program and agency case management training and meetings as requested and required by funding organization and BMHTH

Qualifications:

  • High school diploma or equivalent required, 2–5 years’ experience working in social services preferred 
  • Must have driver’s license
  • Bilingual applicants preferred
  • Candidates with lived experience with substance use preferred
  • Commitment to anti-stigmatizing communication with clients and client candidates

The deputy manager position supports BMHTH’s diversion programs, which serve individuals with behavioral health issues in Walla Walla County. This position reports directly to the Program Manager and supports the general management of our diversion programs. This is a permanent, part-time 20hr/week, non-exempt position with a pay range of $40–$45 per hour, plus BMHTH benefits listed above.

Duties:

  • Analyze program effectiveness and fidelity to the LEAD model for BMHTH Arrest and Jail Alternatives (AJA) and Recovery Navigator Program (RNP)
  • Report to the Program Manager on program goals, objectives, outcomes, and areas for improvement on a regular basis
  • Serve as frontline leadership during Program Manager’s absence
  • Assist the Program Manager with the overall planning, coordination, and execution of program activities
  • Support communication and collaboration among cross-functional teams
  • Support and attend required meetings with external stakeholders on the Operations Work Group (OWG) and the Program Coordination Group (PCG)
  • Complete quarterly reports for the PCG
  • Conduct weekly and monthly audits of all outreach and case management documents including case notes and programmatic receipts to ensure data is accurate, timely, appropriate, and meets contract requirements
  • Conduct monthly audits of client demographic data and documents in Julota
  • Submit and follow-up on all Julota ticket requests
  • Monitor and coordinate completion of required trainings for diversion staff
  • Support occasional special projects as requested

Qualifications:

  • Bachelor’s degree in social sciences, social work, or bachelor’s level degree in another field plus at least 2-4 years’ experience working in social services
  • Fluency with client information software systems, office information systems, and database structures
  • Commitment to anti-stigmatizing communication with clients and client candidates
  • Knowledge of harm reduction and LEAD models of care
  • Familiarity with project and program management analysis and software
  • Must have driver’s license
  • Bilingual applicants preferred
  • Candidates with lived experience with substance use preferred
